The description is incorrect, you need 250 documents read for this achievement.
Documents are everywhere in the game--hanging from bulletin boards, laying on the ground, given by NPCs, etc. Also, when you interact with certain items, that also counts as a Document entry. Documents usually flash when they're not read and when you read an entry for the first time you'll see its name appear in the bottom right hand corner of your screen.
You can see your total Documents Read by pressing
and tabbing over to archive and choosing "Game Record." You can also choose "Document" from the archive screen and see which you're missing. This Steam guide helped me track down the last 30 I needed: https://steamcommunity.com/app/1731070/discussions/0/6164890...

Unfortunately I could not find any video guides on all documents only a text based list. I would recommend playing through the story as normal and collecting as many documents as you come across. In the inventory screen you can view all documents you have gathered and are missing and cross compare them against the list below.

You can choose between speaking up and convince them to go back to black streets, or you can stay quiet. If you stay quiet they will mention going to a relatives house near laurel crafts. At the lion plaza where you had to dodge eulalia's feathers you can go through a ruined house towards the granary outpost. On the way you will see a warewolf and a cultist mage. If you kill them there will be a small object under the rubble that gives the document and a quest item.
