Unlocks after completing hidden episode. In order to unlock this episode you need to get 5 S ranks on any difficulty or play a total of 50 episodes.

To get this you must beat True Episode 18: Turn of Events.

This is unlocked by getting at least 5 "S" ranks or completing 50 episodes. Since that means total episodes played and replayed needs to be 50, it's wiser to go with the "S" ranks. It's really the same episode, just with an extended ending cutscene. Once you reach the result screen this will pop up.
