For this achievement, you will need to find a Corruption Breach area on one of the maps which will show up in red color as 2 swords crossed. For me, I went to every map and never saw this icon but certain actions tended to trigger it to appear. Once you see the icon and go to it, there should be a floating orb that you can press B to acitvate. It will show the monsters on each wave and the Optional Objective which has to be completed. I believe the objectives are random. So, if you do not like the objective: reload, go back to what you did to trigger the event and try again. Once, you finish the event, it will not appear.
The event I used was on The Black Alms Map, mid map you should see a main character icon for Khendra of Bastengar. Talk to her and the Breach event should appear right above you. If there is a Heavier or other monster in the same area, kill that before starting the breach event. The first time I activated the event, the optional objective was to defeat all waves in 90sec. I didn't attempt this but reloaded and got the objective to sand slide 10000 meters. Super easy, I just slide around the outer margin of the event until the counter on screen said objective met, then killed monsters. Easy.

Corruption Breaches are new, open-world events that will not show up on your map until you're fairly close to one. However, the game gives you a quest called "Corruption Breach" right away that will lead you to one automatically if you follow the distance listed on the quest screen. Mine was in the Monsalar map at the location in the screenshot below.
