This achievement should come naturally throughout your playthrough, you will discover locations while traversing the overworld moving between towns. These must be discovered by you personally, locations named/discovered by your companions do not count. Additionally, when you're in proximity of a new location, you must press 
to "Enter" that location in order to complete the discovery. There are well more than 5 locations throughout the game so you should have no trouble finding enough. I have attached a map of all locations below.

Additionally, after fulling your requirements for this achievement, you are able to reveal the rest of your map through the use of console commands if you so wish (or continue discovering them normally of course). Using this before your 15 discoveries for the "Explorer" achievement will not count.
Accessing the Console:
To reveal all locations on your map, use the command "UnlockMap".
