Level 4 ends when you kill the first boss and exit the level. The lasers that the boss shoots do 0.5 damage while the bullets do 1.0 damage.
Get hit by the laser at least once to get a health of x.5 then whittle the boss's health down. Once he is about to die, he will shoot a lot more bullets. Get down to 0.5 total health here if you have not already, then finally kill the boss. Pick up your reward if you can safely, then leave, and the achievement should pop almost immediately.
