I unlocked this by equipping the following, but I'm not sure if you need all 4 of them (and I'll include modifiers but I don't think this is important).
Weapons Type: Laser D+
Volume of Fire: Quintuple Shot Medium D+ P---
Rate of Fire: Long\Slow Ratio P+
Shoot Attribute: Piercing Bullet

I didn't have quintuple shot nor quadruple. I had Laser D+, Triple Shot D-, Long/Slow Ratio P+ and Piercing Bullet. So I would say volume of fire is insignificant and focus on rate and shoot attribute.

Not sure if volum of fire is needed for this one. I had laser, quintuple shot and long\slow ratio one run. But no achievement. A later run I had laser, quadruple shot, long\slow shot ration the achievement popped as soon as I added piercing to it.
So either the volum of fire do not matter, or you can get away with quadruple shot, which seems to be much more common than quintuple shot.
