V. 2.5 is unlocked by dying due to a trap.
The traps are the red (or sometimes orange) tiles you see scattered around some of the levels. Once you find one (preferably early on in your play) you can stand over it and keep receiving damage until you die. You will unlock the achievement during the 'game over' screen.
