There are 12 bottles total.
4 on the well after the first part
4 on the left of the house (you must try and get these as you walk up to house)
4 on the right of the house.
If you miss one, just hit quit and restart.
The achievement unlocks at the end of the stage.

There are 2 bottles that are super missable and they are only shoot-able as you are coming up the steps towards the house before turning right to shoot more skeletons.
The two bottles in question are the ones on the left side of the house (the right 2 of the set of 4). Shoot them quickly as you come up towards the house. If you miss them then you need to quit to the main menu and restart the stage (or let the enemies kill you). The other 10 bottles throughout the area should be easy to shoot as they appear during sections with enemies and you can shoot the bottles before taking out the remaining skeletons. There is a Megashot that you can shoot and equip right as you are going up the stairs towards the house (in the distance off to the right). Use it to make it easier to shoot the bottles.
Warning!: If you die after shooting all the bottles, then you will need to restart the stage from the beginning and shoot all the bottles again. Shooting all 12 bottles, hitting a checkpoint, and then dying will STILL void the achievement. All miscellaneous stage achievements for this game MUST be done all in a single run without ever dying!

For some reason, this is not unlocking for me. Did you complete these requirements while playing solo or with friends?

Near the start of the level, you will see the bottles on a well and then again on a railing. You must shoot all of these very fast. The camera moves past 1 or 2 that you normally could not reach, so be ready to shoot the lats ones while the screen is moving. It is suggested to have a few people helping you out on other controllers.
Note: This achievement unlocks at the end of the level.
