This can be quite tricky to actually pull of. When a Titan tries to grab you, you have a split-second to press 
and dodge out of the way.
Use your momentum to get around the back of the Titan and you should see a
appear on its nape. Press and you will pull off the hook dive.

Does the A appear or we just jump when we feel it’s the right time?

Unfortunately there’s no prompt for the A. You have to jump as his hand is about to swing down to grab you. You can jump in any direction to avoid it, but you have to end up behind or above the Titan for the Y to appear.
I forgot to mention that you have to be locked onto it too, so i will update the guide with that as well.
