This achievement only applies to the main storyline missions in Attack Mode. You don’t need an S rank in any of the Expedition missions to get this.
In order to earn an S rank in a mission, you need to kill almost all of the Titans on the map, complete all the side missions (these appear as a green “!” on the mini-map throughout the mission) and complete the mission under the par time.

I completed all the main story with an s rank and the achievement still didn't drop. Do you need to complete everything with the same character?
