Whenever you enter a mission, if you press start there is an option for "Special Mission Requirements".
Complete these and the hidden Dire Titan will appear. Be careful of time sensitive missions and always take down this Titan before the final subjugation boss.

Is there a dire subjugation target on every single mission?

Dire Subs become available once you finish chapter 3 of Attack mode.
Every campaign mission has one with certain requirements to make them spawn. Once a mission starts, pause the game and check "Special missions" for the requirements.
In order to make your life easier, set the difficulty to Easy in the Main menu (->Settings ->Game ->Difficulty) since most of these are Annie/Reiner-reskins or large titans that sponge a lot of damage even on Normal.
Some missions might be very confusing to achieve as well:
"Idol" for example wants you to have Reiner and Berthold on your team.
In order to get them both to spawn on the map, you need to finish Annies side mission, then proceed to fly around the bottom left corner of the map until dialogue starts and Reiner is marked. Go near him and make sure to enlist him by pressing B. Then proceed to the bottom left corner of the map to find Berthold.
The enlisting thing is important for any of these missions, but usually the spawns are tied to their side-missions which you usually have to do anyways.
The "57th Expedition beyond the Walls" also wants you to "Have Miche, Christa and Ymir join you". In order to even make these spawn you have to fail the first main-objective ( Killing the large 3 Abnormals on the main-road to make way for Erwin).
