Pretty basic stuff. The sixth overall mission called "Roar" features Eren's Titan form. Simply walk up to the first Titan you see and hit either 
for a punch attack or 
for a throw attack. Once you do either one, the Titan should be killed and the achievement should unlock. Easy as pie.
