When you first encounter Plague Spores, they will either be flying towards your colony or picked up by your expedition vessel.

When colonists get infected they will be listed as "sick" within the Colony Control panel. Make sure you disable Plague Vaccine distribution from all Hospitals so the illness will spread quickly. You can always save and reload after achieving this trophy. A Plague can only start, once either Plague Spores attack and aren't destroyed or when an infected expedition returns from destroying a Plague Spore nest.
