When I played the game was glitched.
I believe I played normal, but the game isn't actually that hard combat wise. It only affects the mini boss battles.
They do indeed stack (or at least should do). As I say I'm not sure on the state of the game now but it was very glitchy and buggy with trophies.

There is no Hard in the menu so it probably unlocks afer finishing the game once.
Do I have to play on Normal or Easy will suffice?
And next question - do difficulty trophies stack or are we looking at a 3 playthroughs here?
