Hi All,
Only one secret to be found in Act 5, it is in the chapter - In The Depths. It can found just before entering the safe room by the blockage covering the tracks.

This secret can be found in Act 5 - In The Depths at the end of the level before the safe room.
After repairing the bridge for the third time you'll have to arm the minecart with explosives, before arming the explosives check the corner on the right in-front of the minecart to find the secret, interact with it to unlock the Achievement.
