To earn this achievement you have to enter and exit all 7 Ridden Hives. Exiting a Hive will give you the Completion screen.

A few pointers towards this:
- You don't have to collect the Skull Totems. They're just currency & a melee weapon. You should come across one before your first exit.
- If you go down or die and your teammates exit the Hive, you still get progress (similar to the completing levels on difficulty modes).
- You don't have to play the whole level. You can exit the Hive at your first exit and it counts as a completion.
- Listen to the dialogue of the characters and they'll let you know if there is a Hive near by, and you can hear when a Hive is near by. Listen out for a echo of ridden making noises in a hollow cave.
- At the time of this guide post Act 3 Farther Afield was the level to boost Hive spawns. It has 3 spawn locations starting after going through the corn field. The first, on the left side between buildings in the corner after corn field, the second to the right after you've gone through the house before the start of the treck up the mountain, and the third right at the end of the level on the left side of the exit next to the outside toilet.

At the time of the achievement guide post, players were playing Act 3 Farther Afield. Players were going through the level as the spawn rate of a Hive was very high. Play upto the corn field and that's when they start spawning:
1) After the corn field, on the left hand side between buildings a entrance can spawn. Either jump up the ledges or walk up the road and turn left and you could see one.
2) Go through the house before the mountain slopes, turn to your right and there can be a spawn beside the house. Left/right side of the house depending what end is the front of the house. It's hidden by a fence as you're walking through the house, you see it once you've gone through.
3) Last location is right at the top of the mountain before the end of the level on the left side. Next to the outside toilet.
The characters will say dialogue if a Hive is near by, and you can hear them. Sounds like an echo of a hollow cave.
I was doing this method. Play through, find Hive, complete it, quit out, pick Act 3 Farther Afield, rinse & repeat.
Once finding a group of new players to play with, we were teaming up regularly and running through each Act on Nightmare/No Hope easily doing all the Hives possible.

THE VERY FIRST RIDDEN HIVE IS IN ACT 1 CHAPTER 1. NEAR THE OPPOSITE SIDE OF THE SCHOOL BUS, RIGHT AFTER YOU AND TEAM DROP FROM THE LADDER. IT'S ON THE LEVEL OF, WHICH COINCIDES WITH ANOTHER ACHIEVEMENT, KILL ENEMY AND DESCEND LADDER. THE RIDDEN HIVE IS RIGHT BEHIND THE BUS , WELL, NEAR THE ENGINE MANIFOLD, WHICH IS THE FRONT OF SCHOOL BUS, THE DRIVER/PASSENGER- SIDE.

If you're just trying to find one or two of your missing hives, I'd suggest bookworms from Act 1. My partner, Godshooter1, and I were able get our remaining hives by replaying bookworms with speedy characters. I'd suggest Karly and Evangelo. It took about 2 minutes on the easiest difficulty to board up the library, then there are 3 spawn locations it could be:
1.) from out the door, near the other side of the bus on your right
2.) Continue up from the previous hive and it should in the alley
3) last possible spawn is near the survivors, outside on ground level
If it didn't spawn in any of these 3 locations, restart. In total, each run should take about 5 minutes, just continue replaying until you've found the hive location you're looking for and thank the RNG gods. Good luck!
