The secrets are Golden Skulls and there are 10 total hidden throughout the game. This Secret can be found in Act 1, The Dark Before The Dawn, Special Delivery. Make your way to the end of the level and when facing the safe room turn to your left and you should see a stack of pallets and the secret will be on top of them.

Hello Everyone!
This can be accomplished in ACT 1 Chapter "Special Delivery." Shortly after leaving the safehouse and progressing naturally, you will come across a red flare leading to a building you can walk into. Progress a little further until you read a wall that you can climb up with a bunch of pallets stacked on top of shelves. The Golden Skull collectible is on top of one of these shelves. Please see the attached video if you prefer visual representation. Good Luck!

Back 4 Blood collectables include Golden Skulls with 10 hidden throughout the entire campaign. You can't collect the Back 4 Blood Gold Skulls in single player but this one can be found during Act 1, The Dark Before The Dawn - Special Delivery.
Toward the end of the mission, before closing the safehouse doors once the two supply crates are collected, turn around and look on top of the pallet shelving. Climb to the top and press X next to the Gold Skull to collect it and have the achievement pop!
