*Small Aside: I am currently writing this at a McDonalds as power lines near my house were cut in half following a storm. Some information in the solution might not be 100% accurate due to not being able to double-check this either in game or not having time to cross-reference. I plan on editing this once power is restored.*
Skull tokens can only be found within a Ridden Hive. These are an optional mini-area which can be toggled on or off for spawning when a new campaign is created, however the default is set to on. They are a randomized area which can be entered when all players enter the entrance to the Hive (this gets pointed out by either other players or NPC's when they get near the entrance, however I haven't played a whole lot with other players so I could be wrong. One of the new playable characters automatically sees these entrances which I use as a bot, so take this section with a grain of salt.)
When in this area there are multiple exits along the progression of the hive, and each segment has a unique weapon called a skull totem. The rare ones I have seen are accessible by jumping to the other side of a bottomless pit. The totem does take up a primary weapon slot while it is being carried, and you will have to carry it while exiting the hive in order to gain both this achievement and as a form of currency. Exiting the hive finished both the hive as well as the map the hive was found on.
As a bonus there is also an achievement for killing 10 ridden with a skull totem titled Using Your Noggin, so rolling with only bots on Easy should make this a little easier to get through.

can be done in trial of the worm as long as right card comes up, only had to carry 1 totem to mission end. don’t own dlc

Each Hive has 3 finadable Skull Totems (Which can be combined). Depending on the levels, these can be missable as most hives contain drops which cannot be backtracked. To earn Skull Totems as currency, you need to find at least one totem and keep hold of it at a level exit (some hives contain multiple exits).
