Back 4 Blood

This secret can be found in Act 4 - The Abomination. After destroying the first set of weakpoints on the boss it will start to crawl away, eventually you will come to a big open room with tentacles that form multiple bridges and the secret will be on the tentacle going to the right into the wall.

#10 - (6:54) - ACT#4 “The Abomination” Secret - Part 1/1 “ABOMINATION”

- This Skull can be found during the 3rd stage of the final boss battle. I found it was easier/less stressful to run ahead of the big slug until you get to a sort of tentacle bridge, the skull is wedged into the slime at the top of the first "branch" on the right.

Each Secret will unlock it's own achievement, there is no cumulative achievement for getting all of them.

Secrets are golden skull like trophy things, you need to interact with them by pressing x to unlock the achievement. There are 10 in the game in total.

This particular secret is located in Act 4 - Abomination. It is shortly after Abomination starts to crawl away, you will need to cross a bridge and take a right up to the secret.

In this video guide I will show you how to find the secret in "The Abomination". This can be found in Act 4, level called The Abomination.
This golden skull won't be until you are chasing the boss in big open rooms. The third open room you get to will be the room you need. You also might need to wait for the boss to move up a bit before being able to get it. Once the boss moves up enough there will be one of the tentacles you can climb up on the right hand side of the wall to get the golden skull.

Hello Everyone!

This can be accomplished in ACT 4. After completing the second phase of THE ABOMINATION boss battle, you will have to chase it and shot the weakspots as it is running away. Continue a little bit past that point and you will come to a more open room with multiple tendrils reaching from side-to-side. The Golden Skull collectible is on the right hand side tucked away at the end of where one of the tendrils meets the wall. Please see the attached video if you prefer visual representation.
