This is how I got this achievement:
When you receive a kickoff, run back towards your endzone and run out of bounds right before you cross the goal line. This will cause your QB to line up in the endzone on offense. Choose “Empty Backfield” then “Hail Mary” and throw to the Y receiver (should be Pablo if you’ve played Story Mode). Hold RT to sprint and use RS to juke or plow through the defenders.
And on my second try, The achievement popped!
