I got this by following the bad ending route. Go all the way to Chapter 8, and once you start the chapter, immediately click on the ladder that leads back up to the house and then click on the doorway in the picture to leave the parent's bedroom.
There is a battery in the toy car in the child's room and film near the TV on the floor in the living room. Then, go back down to the basement and enter the graveyard by clicking on the tombstone in the drawing on Death's table.
Talk to the kid and he will disappear and then cross the bridge on the right and enter the house with the key. The camera should be sitting on the table. Put in the film and the battery and then pick it up and you should get the achievement.
As a note, it unlocked for me but it didn't show up on my screen for some reason. I don't know why it did that but maybe if you don't see it pop, check your unlocked achievements just in case.
