You have to be doing this in a perfect good karma run.
You find the dirty rag in the outhouse behind the old house in chapter 7.
You will need to wash the dirty rag in the lake to the left of the pier. You will need to place the rag over a doll's eyes to go into the rooms but before you enter the basement to finish the chapter, grab the rag off of the doll's eyes.
In chapter 8, make it back to the apartment area and go downstairs to find a key on the ground. Use the apartment key on the right apartment door. Inside, the mirror will be dirty and you need to clean the mirror. Grab the soap spray from the right cupboard in the kitchen and then use the soap spray and the clean rag to clean the mirror and net yourself another achievement.
