You must smash the doll with the crowbar for the wire in the first chapter.
In chapter 2, you will need the key to the bathroom, which is in the cafeteria. You will need to talk to the lunchlady in the waiting room, who will give you the key to the lunchroom. You need to go in there, grab an empty jar and capture the spiders in a jar and give it to the lunchlady. Then you can go back into the lunchroom to find the key to the bathroom in front of her on the counter.
You will need to flush the toilet in the bathroom to get a key to the doctor's office. It will be in the toilet after it backs up. On the floor, will be a worm to get the eye from the fish that is at the reception desk. Also, grab the bottle cap from the floor and the hair from the sink. Make sure not to use the key on the doctor's office.
Trade the bottle cap with the bird in the elevator/vending machine room and give the worm to the fish for the eye. Place everything on the patient in room 15, then go back to the window where the bird was and collect the other eye from its nest.
Finish the level and continue through to the forest. Use the doctor's key on the cottage in the woods for the achievement!
