I did this fight several times outside and still didn't get the cheevo. The traps by the wall were going off maybe by aoe damage or thru the wall. I ended up before the fight just shooting and destroying all the traps in the room with the ladder with my bow and then did the fight outside and it popped. So if your struggling destroy the traps first it doesn't penalize you.

Goes wthout saying but you definitely want to make sure that you have disabled the Steel Watchers first before you even consider attempting this!

There's a really quick and simple way to unlock this that doesn't require you fight Gortash or any of the Steel Watchers.
We'll be using the Improvised Melee Weapon action, so best to have either Laezel or Karlach in the party, as there the best bet to have enough strength to pick up Gortash.
Once you get to Wyrms Crossing, go to the inauguration and agree to ally with Gortash. Agree to stick around and don't interrupt the ceremony.
Once this is complete he will be standing there at the throne. Move your character towards him, select Improvised Melee Weapon and select from world. Highlight Gortash as your weapon, so you will pick him up, then move your cursor to the other end of the hall (where you came in by the big door) and press A again.
Your character will now pick him up and wander to that location. Whilst they are moving, quickly press the back button and Y to access the fast travel waypoints and teleport to South Span of Wyrms Crossing.
When you appear Gortash will be stood outside by the way point.
Repeat the process of picking him up, but this time we're moving him to the lower wooden platform to the east, overlooking the edge of the bridge. We want to get him as close to the edge as we can.
He will get thrown to the floor here, go prone for a tick and get back up. He will then stand there waiting about enjoying the view!
Now all you need to simply do is use the Shove bonus action on him - making sure he will fall over the edge - and he will fall to his death.
You will then get the achievement and can go down to the shore to get his loot!

Then use area spells, i used Insect Swarm and Cloudkill, down the ladder hole, he will stand in those areas taking damage without even moving. I threw an elemental summon to where the broken ladder would have been to kite him and his guards towards the hole, and could use ranged weapons to pick them off from above, or leave him to die from the spells.
Bring most of your team to the roof except the ranged, have them attack one of the guards in the room with Gortash. It took 2 hits to aggro the guard, and run back up to the roof after the second hit. Break the ladder to the roof as soon as everyone is up there and Gortash is on the second floor, and he wont be able to hit you anymore. And since there aren't any traps up there to be activated, this achievement popped once Gortash dies.
This video only shows the set up, but afterwards you can simply skip every turn until he dies or try to hit him from above, either way he stays in the fields of damage.

In the "Tactics" mode, damage from traps in the hall during the battle was counted and the trophy did not drop. I had to take three scrolls of the “sphere of invulnerability” and stand for the whole party inside.

The trophy dropped the second time, although I definitely didn’t activate any traps (either in the hall or outside) and didn’t receive damage from them. I killed it the first time - nothing rang. Rebooted, killed again, received the trophy after Gortash died.
