Saving Gale
Save Gale from a portal on the Ravaged Beach where you land after escaping the ship at the start of the game. Then, he will periodically ask that you give him a magic item that he can drain to satisfy his condition, so keep a few magic items on hand that are not needed. Over time he will reveal more details about what happened to him.

Repairing the Weave
As you travel toward Moonrise Towers via the Mountain Pass, you will come across a weary traveler, Elminster Aumar. Invite him to speak with you at camp and he will give Gale a Charm which will stabilize the Netherse orb. Doing so will unlock the Repairing the Weave achievement.

Destroying the Netherbrain
After defeating Ketheric Thorm at the top of Moonrise Towers, make sure Gale is in your party before entering the Mind Flayer Colony via the tower in the northeast corner of the roof. Head straight toward Ketheric Thorm in the far east end of the colony. During the cutscene, have use the Netherse Orb to end the Netherbrain.

If Gale wasn’t in your party or you missed the opportunity in Act 2, this can also be done at the end of the game. Make sure Gale is in your party when you get in the boat to travel to the Upper City for the final sequence in Act 3, and he will offer to use the Netherse Orb there.

The only thing I will say is that Gale doesn’t have to be in your party at the end of the game to get this achievement.

Never once used gale the whole playthrough since dragging him out of the portal, just followed the leave no one behind achievement and this unlocks at the end without worrying about dialogue

You just need to follow the dialogue to destroy the Netherbrain at the end of the game. You can use Gale to do this but it is not required and you can simply take control using one of a few methods and then use it to take out the mind flayers and save Baldurs Gate. Reading this I was thinking it was tied to using Gale but it isn't related although doing so in Act 2 does complete it early rather than end of the game if that was something you wanted to do.

Yeah if you want to knock this out early at the end of Act 2 instead of the end of the game, just make sure Gale is in your party before you enter the Mind Flyer Colony.

I just wanted to add that the achievement will pop during the end credits (if you sacrifice Gale to destroy the Netherbrain).

Super easy trophy. At the end of Act 2 when you first see the Elder Brain, if Gale is there he will say something like "this is my moment to destroy the Absolute!" Save your game, and let him. Pop.

If you wish, and just for variety, you can get it already at the end of the second act.
We just take Gale with us to the final battle with Ketherick and during the cutscene there will be an option to blow himself up.

If at the end of Act 2 you take Gale with you, go to the Netherese Brain and let Gale explode the sphere, then the trophy will also drop out (the trophy for completing the game and for the difficulty level does NOT drop out).
