The enemies death must be caused by falling and landing on solid ground of into an abyss. Rivers don’t count. This can easily be acquired by going to the Rosymorn Monastery and walking up to the edge. Next, ungroup your party, place one character on the edge, and a second character next to them on the monastery-side. Then, cast Thunderwave to push them far enough off the edge that they cannot land on the ground below.
Any character can use the shove bonus action, but high strength and proficiency in athletics will help. Open hand monks and swords bards have special moves to knock a character back. Archers can fire a special thunder arrow that knocks enemies and allies back, too. Also look for chances to destroy a platform that an enemy is standing on.
The problem is that falling only does a small amount of damage and an endless fall won't get this achievement. Work on this with injured enemies in the early game.

Oddly enough, I earned this achievement in Act 2 by jumping from a second floor landing onto an enemy as a wild shaped druid.
