The Surgeon/Malus Thorm can be found in Act 2.
He can be found in the House of Healing flanked by four nurses.
I’d recommend approaching him and initiating dialogue, with speech checks you can persuade the nurses to kill each other, making this significantly easier.
To perform Surgery he needs tools, these can only be gotten from the nurses (and only if they’re alive). He’ll occasionally try to resurrect a nurse, simply focus it/them down asap.
Finish combat without him receiving a weapon from any of the nurses/performing surgery and you'll nab this achievement.
This CANNOT be gotten my using speck checks to have him kill himself.

The surgeon, Malus Thorm, is one of several boss enemies you can find around the darkened expanse of the Shadow-cursed Lands of Act 2. He can be found in the House of Healing performing surgery with his four Sister nurses. For the sake of this trophy, while in combat, he will perform an action on one of the four nurses around him called Surgeon's Choice. You now have to make sure that they do not hand off their preferred tool of choice to him, either by keeping them from moving via entangling or other mobility-hindering abilities, or just simply defeat them. After he has run out of nurses, he will attempt to bring them back, so it can waste one of his actions. Finish combat without him receiving a weapon from any of the nurses and you'll obtain this trophy.
Caution: when he loses about 40% of his health, he will start doing a Multiattack that can deal severe damage if unprepared.

I noticed if you succeed on one of the two Persuade checks, the 2nd one IIRC, the nurses all kill each other and you are left with just the boss. Now he can and will use his turn to resurrect one nurse. Then he will use the next turn to ask for a surgical instrument. But I found the resurrected nurse had about 1 hit kill worth of health and could be dealt with quite easily.
The other Persuade check actually has the nurses kill him, but the trophy does not pop. No fight and no trophy.
If you fail either Persuade check, then you take them all on. I tried taking them all on at once a couple times but struggled pulling the trophy off.
Next time I play, I'll check the wording of that Persuade check and post it if you like.

I've been able to successfully use this strategy three times on both balanced and Tactician, so sharing it here for anyone struggling against him.
This should be done with any character in your party regardless of class.
-Savescum (or get lucky) and have the nurses kill each other, text choices below:
>any of three initial choices you first get (do not choose attack, you'll have to fight him and the nurses at the same time, making it harder) should work.
>Choose any of the persuasion options (One does have the nurses kill him, "They need a better subject to practise on first. Not a student, but a master", but it does not trigger the achievement!)
>Once the cutscene of the nurses killing each other finishes, you have the option to persuade him to kill himself or attack him. Choose the latter, as I previous mentioned, the former option will void the achievement.
As for the strategy:
I have two characters on him doing damage, and another taking out whatever nurse he resurrects. If you have ShadowHeart or another healing character, I recommend keeping them station above where he can't target them, but can easily heal and provide some damage doing the fight.
This should be a simple and effective method to prevent him acquiring any tools from his nurses to do lot of damage and grant you the achievement.

Double checking the fight to ensure I have the correct information: Regarding the second choices of dialogue, they can change depending if you've encountered certain pieces of text. I written a list down below of dialogues that have void the achievement (cause the nurses will kill him):
