Simply play the Dark Origin character from the get go. You have to give in to the urges and serve your evil "father", and then take control of the Netherbrain in his name. This will be a dialogue option after the final battle with the brain.

Its worth mentioning the three main plot points for fulfilling this playthrough are, you will be asked by Bhaal to:
Kill Isobel
Kill your closest companion
And then fight a duel with Orin to claim your place as Bhaal's chosen.
In my playthrough I ignored the first two requests, and then during the duel with Orin I still brought my other companions into the fight. As long as your main character is not killed, Bhaal will not care.
And then obviously at the very end of the game make the Bhaal dialogue choice.

You can resist the Urge partially and still get the achievement.
