How to unlock the Ceremorphosis achievement in Baldur's Gate 3 - Definitive Guide
This achievement is obtainable in the final part of Act 3. Spoilers ahead (obviously).
The first step happens when you get to the Astral realm. You can choose the option to betray the Emperor and free Orpheus, or stay with The Emperor and not free Orpheus. Either way, the dialogue will get to a point where someone can take the parasite to become an illithid to take down the Netherbrain. There will be an option for you to sacrifice yourself and become the illithid — you must choose this. (*Note - if you have Karlach in your party, she will try to take the parasite herself multiple times. Make sure you always select the option where you refuse this).
You will then transform into an illithid, and you must complete the entire final battle sequences as your new illithid self for the achievement (both courtyard sections and the final boss fight after climbing the brain stem).
Once you defeat the Netherbrain you are brought down to Baldur's Gate for the final cutscenes and dialogue options. If you are planning to enjoy the story and the epilogue, choose whatever dialogue options you want. If you are looking to get this achievement quickly, do the following:
Your achievement will pop during the rolling credits – don't panic if you are still seeing cutscenes and have not gotten the achievement yet.

I haven't completed the game as an illithid yet but I'm curious to see if what you say is true about HAVING to betray the emperor. If you ask him if your able to use the stones, he will end up turning your into an illithid himself.

After failing in your first attempt to control the netherbrain with the stones, the Emperor will save you by pulling you back into the astral Prison. He will suggest that he come with you and use the stones, but he’s pretty weak, so suggest you become a mindflayer yourself. He’ll tell you to talk to your companions about it first. Most will be against the idea. Karlach will suggest herself to become a mindflayer instead of you. Ignore them. Become a mindflayer and complete the game, killing the netherbrain.
However, as a mindflayer, you lose some abilities. Your strength and dexterity drop to 13, and your Intelligence and Charisma become high. As a mind flayer, you can use bonus actions as actions and vice versa. A build like thief 3/wizard 9 will make great use of these changes, while a pure barbarian will be nearly useless. Thus, if you plan on this achievement, before you get in the boat, talk to Withers and change your class. The Thief rogue’s two bonus actions are fantastic for mindflayer PCs. Other good builds include:
Frontliner: Fighter (Eldritch knight)3/ Thief 3/ Warlock (blade) 6
Healer: Thief 3/Bard (lore) 9
Caster: Fighter 1/Wizard 11

I got it under the following conditions:
- turned into an elite
- killed the brain
- went to hell with Karlakh to kill the demons

My hero did not turn into an illithid, in the end Karlakh became an illithid and they gave me the achievement anyway.
