If you have already played the game, you'll have far in excess of 150 deaths. Simply get one more death for all 5 achievements to unlock in one go.

I'm not sure if my progress didn't save or if I accidentally pressed new game. But if anyone else has to start from scratch like me, the easiest thing to do is to beat the first 3 levels and then rubber band on level 4. You just have to go right into the spike
