Note - gold medals earned in DLC levels don't count. To unlock this achievement, you must get gold or platinum medals on all 20 story missions in the "Maal's Story" campaign.
To get a gold or better, you must acquire enough points. You score points based on the time it took you to complete a level, how many turns it took, how many enemies you killed, and you lose points for each of your own bugs that die.
Getting a gold is usually pretty straightforward once you're familiar with each level. Bear the following points in mind:
(1): Don't sit around thinking too much - the quicker the better.
(2): Try not to waste any moves - the fewer moves to complete the level, the better
(3): Avoid having any of your own bugs die AT ALL COSTS. Use your Healer, and healing items if necessary.
(4): Kill as many enemy bugs as possible, but as a secondary objective. For levels where you have a different win-condition, focus on fulfilling that condition, and kill any enemies as necessary.
EDIT - Note, if you finish a playthrough of "Maal's Story", you can just replay the individual levels you didn't get a gold or platinum on.

Thanks for confirming that the medals must be earnt in the "main" storyline missions :)

I used a FAQ on Gamefaqs to get this achievement. There is only one by ThermalStone. He describes every mission perfectly and explains how to quickly and efficiently win the mission. Finished most of them with Platinum Medals that way!

Complete the storyline with all platinum/gold medals. Completing each mission with a minimal amount of turns as well as a fast time will always give you at least a gold medal. If you follow the guidelines I gave for Let Loose the Bugs of War, you will easily be able to get the gold the first time around, at most second.
