'Complete the Hanging Gardens and speak with the Survivor in the Bastion'

Near the end of the Hanging Gardens level, you will find a man standing in the middle of the path. Press X to speak to him and end the level.
After a cutscene, you will both be teleported back to the Bastion. The achievement will pop when you talk to the survivor in the Bastion. Not just after watching the cutscene.

Mostly story related.

You will need to rescue the Survivor at the end of Hanging Gardens. After that, just talk to him at the Bastion and the achievement will unlock.

This is mostly story related, but you can technically miss it if you do not speak with Rulf when you return to the Bastion. You return many times after rescuing him, so it doesn't have to be right away.

Story-related and cannot be missed.

You'll meet up with the Survivor, Zulf, and rescue him in The Hanging Gardens, the 7th level in the game. Once you've completed the level, simply talk to him once you're back in the Bastion to unlock the trophy.
