The guy fails to mention in his videos on how he is able to use the triple ultra batclaw without first holding LT to target the enemies or how he's able to already have it selected when the round starts.

He doesn't need to. Double tapping LT is enough.

To add to David Hartly's solution, if you're having problems getting the necessary points to get three Bat Medals, try doing ground pounds, which is a takedown move where you knock down one goon, freeflow to the next goon and then pressing LT+Y to do a jump towards the downed goon and performing the takedown. The points earned for this maneuver is the current combo streak X 100 points, so be sure to use that when you get your combo up to 10+ hits in a row

You can obtain up to 3 medals on each challenge based on your score at the end of the challenge.
Tips for achieving the highest rank:
- Use as many different combat moves as possible to obtain a higher end-of-round score. These are: Strike (Left click), Counter (Right click), Cape Stun (Middle mouse button), Evade (Double tap space), Throw (Shift and left click), Take Down (Shift and right click), Ground Takedown (Ctrl and right click), Batarang (Q) and Batclaw (C)
- Keep aware of when you have enough of a combo to use Throw and Take Down so you can make sure you've used them as soon as you can
- Continuously evading doesn't break your combo as long as you are jumping over an enemy, so make use of space to avoid being surrounded
- If towards the end of a round the last enemy / enemies are quite far away, make use of the Batclaw
- Try to complete each combat challenge in a continuous combo for a higher end-of-round score
- Try not to take any damage for the whole challenge for a bonus to your score at the end of the challenge. This also applies to individual rounds, but pales in comparison to the bonus of not taking damage for the whole challenge
- A few mistakes aren't the end of the world, so just try and recover as soon as you can and get the combo going again
- Make sure you have enough time to use the Ground Takedown when using it as it can take a while, relatively speaking, and it leaves you vulnerable to attack
