How to unlock the Flawless Freeflow Fighter achievement in Batman: Arkham Asylum - Definitive Guide
First and second wave are really easy, just avoid extreme combos and use Ground Pound (RT+Y) only if other thugs are far enough.
3rd and 4th wave:
- if two thugs come close at the same time, use quick batrang to stun one and hit the other with X
- use Y to counter just when you can see the lighting on the head. Hit Y just one time, or you'll be exposed to the attacks
- Where the combo counter became yellow, you can use instant KO. You're untouchable for a few second, so you can hit another after the KO is done: choose the farest one, no matter the distance (Batman will reach him anyway)
- Use RT+Y only where there's 2 or 3 guys left
- If you're facing an ennemy and another guy is hitting you, use A or double A and restart from a safer position (ex. X, X, A, A, quick LT, Y, X...)
- After a combo or an istant KO you'll probably have to face a couple or even 3 guys at the same time. Use Cape Stun (B) to gain precious time.
If you're be hitten... restart the challenge and resist the wish to break the controller *LOL*
Can easily be done on the first combat challenge room, "Intensive Treatment". Simply attack and evade. You'll know if you've done this right as you will be getting the Dark Knight bonus each round (500 points), then Perfect Knight at the end of round 4 (5000 points).
For a visual walkthrough, see the following video:

Make good use of your Batclaw and Batarang, keep evading, and watch out for enemies which may attack you from behind.
This will either come with practice or patience. Play through the first challenge map "Intensive Treatment" there are less enemies and they won't be armed with any weapons. keep your distance and use some of the tips provided in the videos for Freeflow Gold, they may be for the extreme maps but they will give you some guidance on techniques for taking down enemies quickly.

I recommend using the evade maneuver and throw move a lot for crowd control. This game annoyingly likes to have as many goons swarm you as possible, and unlike later games you can only counter one enemy at a time, so if it looks like you will be attacked by more than one goon, move away as quick as you can.
