How to unlock the Mano-A-Mano achievement in Batman: Arkham Asylum - Definitive Guide
I missed getting the cheevo during the main story missions so I loaded up the Rumble In The Jungle challenge. After a few rounds of fighting enemies it came down to just Batman versus a Titan. I placed a few explosive gels on the floor as time and the Titan permitted. Then, after the Titan charged, I jumped out of the way and triggered the gel. That caused the Titan to be stunned (the same as if he was hit with a Batarang). Once stunned you pummel the Titan until he throws you off his back. Repeat a few times until the Titan is down. Achievement unlocked.
To use this method you'll need to have upgraded Batman and unlocked his "explosive gel", what you'll be doing is taking advantage of the titans obvious attack pattern and placing the explosive gel on the floor making sure to detonate it as the titan runs over it which will stun him, once you've got him stunned just lay into him with your hand-to-hand combat moves and you shouldn't have a problem killing him off.
If you manage to take down a bar of his health before he stands back up then keep attacking and you should be able to kill him before he ever stands up again, if he does manage to stand back up, just lay some more explosive gel on the floor and detonate it to stun him again, repeating this process as you need to.
Make sure you don't use your batarangs, doing so will inflict damage on him, I'd advise just sticking to explosive gel and hand-to-hand combat until you've got the achievement, after that kill them however you want!
The video I've linked in shows this method being used in a combat challenge, it can be used against any titan enemy in the game.

For clarification, there are only 3 spots in the game in which titans appear, so the achievement is missable. The first is in the youtube video below during your first encounter with the titans - when Joker injects them with venom. Now, playing through the game on hard, I found this incredibly difficult to do, so I didn't do it at this part. For those who can, the guide below shows how.
The second titan appears before you fight Ivy and the third (two of them) as part of the Joker boss battle. Since the third has too many guys to really make it doable on hard, try the second titan.
It's the only encounter when there's only one titan and there are electric fences to stun him if you time your dodge right. There are guys that fight you as well but I generally ignored them until the titan was dazed, then just used the titan to attack them.
It isn't easy but you have to position yourself in front of the electric security doors and dodge last minute as the titan charges to make him collide with them.

I used the Batrang only on one of them and than used this titan to beat another one. They are easily to take apart through their color - one is much brighter than another.

Once you start fighting the Titans make sure they run into each other and dodge out the way. Once a Titan is worn down enough and holding it's head start attacking. Make sure to evade side swipes, you may need to attempt this more than once. You don't need to kill both Titans just one.
