Can you use DLC challenge maps

Nope, DLC challenges don't count. You have to get 3 medals/bats on every challenge that came on the disc.

So I'm returning to this game after some years; I have the achievement for the Riddles but not the gamesave. If I now complete all Predator, Combat challenges and do the campaign will this unlock?

Or do I have to do all the riddles again?

You also need to get all upgrades. Sure, most people will unlock all upgrades before finishing the game, but you still need them.

Does difficulty change the challenge maps. For example if I beat the game on hard and go to play the challenge maps are they going to be a "hard" difficulty or are the difficulties of the challenge maps the same regardless of what you played the game on?

^^to the guy above. . .you count the medals in challenge mode, when you are selecting a challenge in the upper right hand corner there will be either 1, 2, or 3 bats, the bats are the medals

I had 1k nice while ago, and I have to say that it is my favourite achievements yet. Not to easy, more or less challanging. 24 medals is V.Hard, I spend 50% of time trying to obtain them.
96% for this game from me!

You have to complete the story related achievements:

After unlocking all challenges you have to complete them as well:

To earn this trophy you must earn 100 percent on one save file. Completing hard mode is NOT required for this trophy although you will earn a trophy for completing story mode on hard..

You must complete story mode, find all riddler trophies and riddles within story mode. Earn gold on all base game challenge maps for combat and predator challenges. Remember for the challenge maps, it is NOT how fast you complete them, just simply complete each one with 3 medals for gold. The only combat challenge that is timed is Shock and Awe but as long as you've got the fighting system down then you should be fine.

To achieve the Perfect Knight rating, you'll need to have done the following:

Beat the game on ANY game mode (though to avoid a NG+, I recommend starting on Hard).
Solved all 240 riddles (See Crack The E Nigma for more information).
Unlocked all 20 upgrades that are found in the Waynetech tab.
Gained all 3 medals on all combat and predator challenges (See Freeflow Gold and Predator Gold for more information).

This will most likely be your last trophy in the game, and where you will be spending the most time, especially the challenge rooms and collectibles.
