Fairly early on in the story after getting the Savior achievement, you will track in on Joker's radio signal. The closer you get to the Joker, the distance on the signal will decrease (or you can just go to your map and look for the steel mill in the bottom right hand corner of the map...this is your destination). The quickest way involves a lot of grappling and gliding. It is best to sail over the gates of Sionis Industries on your way in since the guards are all armed. Once the distance on your radio signal is down to zero or close to it, Alfred will talk to you about having to find a way in through the main chimney. Look up at the big tall chimney and use your grappling hook to get into it. Once inside, you will have to navigate around the inside until you get to a ventilation system leading to a room full of Joker's henchmen. Once you can see them through the grate, the achievement should pop.

You don't need to see the whole thing.
As soon as the Medical Center building blows up you are given a signal to pinpoint. This signal leads towards the Sionis Steel Mill. Once you arrive there, you have to find a way in and the only way is to go through the main chimney. After doing so, you have to jump across to the area in front of you and continue down that path. You will have to sprint under the pipes, climb several pipes then climb even more pipes. After passing them, you need to get out your batclaw and shoot it at the vault filled with water to wet the iron fence. This will allow you to go down to the area where you have to go down and hold yourself, then climb to the next bit. This next bit requires you to go on the iron fence and jump on the other, this is pretty simple. Soon after, blow up the floor and go down there. Navigate through the area until it says to pull out your batarang and aim it towards the button. Pushing the button, you go into the vent and go through it until you see a bunch of goons through the vent. You should have the steam achievement now.

Story related, and cannot be missed.
Upon reaching the Sionis Steel Mill in the Industrial District, you will be greeted by another predator type area with armed thugs. Clear them out, then grapple up onto the Chimney. Once you reach the Loading Bay inside, the trophy will pop.
