Check which islands still have bombs by going into your most wanted tab and then use this map. But I also suggest beating the game before getting these, Because some of them don't unlock.

#15 is in the wrong spot. It should be at the intersection to the northeast of #4

That's true, thx for posting this. Was looking for #15 in the place marked on the map and couldn't find it. However, the rest of the guide is fine (though it would be good to include a node about the real position of #15).

Bomb #4 is in wrong place. It's more to the east this road, near the intersection.

The 94% figure isn't useful anymore since DLC can make you go past 100% without finishing the main story. I'm over 110% and still waiting on the last of the ordinance to show up.

This achievement is bugged for me. Didn't pop either when I took out the last explosive, or when I defeated the militia leader. I'm on 120% completion now and still nothing. It's not even showing as "done....unlocking"

The final road bombs do not appear on the map until almost the very end of the game. They will appear after the attack on GCPD, my main story progress was at 94% (to check this open your side mission list, right on the D-pad, the ! at the top is your main story progress). They appeared on my map after I got this achievement:

Some bombs don't appear until late game (about 94%)

On the map, it should be numbers 12, 10, & 14

In order to obtain this trophy you must destroy all the red blinking bombs planted in the streets around Gotham.
Some of the final ordinance don't unlock until you complete a specific task:

When you find the ordinance, you must attach your wire to it and press cn_X
to install a virus, then you must take part in tank battles with varying degrees of difficulty.
Once you complete the battle you must hook back up with the ordinance, rev your engine and make it explode.

Here is a list of all ordinance locations
Bleake Island Locations: - (2110,-2930) (1800,-2420) (1910,-2670)(2360,-2900)
Miagani Island Locations - (3280,-2200) (3000,-2300) (2900,-2730) (3050,-2920) (3160,-2730)
Founders' Island Locations - (2140,-1910) (2040,-1570) (2270,-1540)
(2750,-1740) (2940,-1450)

This achievement is earned for completing the "Campaign for Disarmament" Most Wanted Side Mission.

This side mission will open up as you progress through the story, the militia have deployed a number of land mine type devices throughout the three islands of Gotham. Their locations will be uncovered as you travel around the city. To disarm them you need to upload a virus to them, defend the area from enemy drones as the virus is uploaded and finally overload them using the power winch. Destroy all the mines to unlock the achievement.

Bombs are in the ground. You will need to defuse the first bomb according to the plot, so don’t miss it. Next, the map shows a counter with bombs in a certain area.
