How to unlock the Free Flow Fifty achievement in Batman: Arkham Origins - Definitive Guide
What you need to do is to ultra stun the Enforcer (with B,B,B) and the spam the X button to attack and counter with Y when an enemy is going to hit you in the middle of your combo.
When I did this my combo was 76x which was more than enough for this achievement (50x combo) so you could have a margin of error before you reload your checkpoint to try again.
If you happen to have the Deathstroke add on pack that gives you the challenge pack 1vs100 load it up and put on the perk to punch through armour shields and taser-batons. When you start the match start your free flow and get a 50x combo. If you don't have the challenge pack the other method works just as well.

Get a 3x combo to enter Free Flow Mode which increases your strikes range allowing you to flow between enemies to knock them down with each strike. You need to also counter enemies with Y because they can end your combo if the hit you.
I just got all the combat medals for 4 characters on City two weeks a go, yet I'm struggling with this! The camera angles seem off, Batman seems to stop being responsive mid fight, and I guess using counter is a must as I keep getting twatted mid move if I just evade?
I'm trying it against Penguins men on the boat. There seem to be enough henchman at this point in the in the natural story progression. If he needs upgrading, maybe that's where I'm going wrong?

When you meet the first Bird gang there will be ~20 fellas to take care of and they wont be tough ones, so you the gloves and the achivement will be yours. I never get more than x20-25 during the game.. But this time i got x85. Good luck!
You can also hang about "Sheldon Park" until a "crime in progress" appears then go for it there.
I have added another video which shows how to get an easy 50x combo on one of the training programs.
After the fight, all of the henchmen in the room jump in the pit and you must defeat them all to progress. Take your time and MAKE SURE YOU COUNTER ATTACKS. Do this to keep your combo up.
EDIT: Don't use any combo take-downs, get into Freeflow Focus Mode so that you move faster and strike harder.

I got him pinned in a corner, hit B, hit X 6-8 times, then B, and X 6-8 times, and repeated until the combo hit x50.
Don't get x10 hits in one go as the combo will be broken by Croc.
If you've played the previous games in the series, you should be familiar with the combat. If you get hit, blocked or you go too long without hitting anyone you'll break your combo. This isn't as bad as it may seem, as you will receive shock gloves through story progression. Once it is charged, and you activate them with +, it is impossible to be blocked, and every hit adds x2 to the combo (x3 if it is also a critical). As long as there are enough enemies this enables you to easily get this trophy.
This achievement is best done on Round 4 of the “Intensive Care” Combat Challenge Map, as you will have plenty of enemies to fight and it is unlocked early on in the game. All the enemies on this particular Map are standard goons, so you will not have to worry about armored, shielded, or weapon-wielding enemies.
The best tactic in raising your combo meter to 50 is to hit an enemy, then jump over him; hit another enemy, and then jump over them. If you keep repeating this process, you should be able to take out all the enemies with ease whilst never breaking your combo. Additionally, if you have unlocked the Shock Gloves in the Story Mode, activate them during the Challenge Map to get a x2 multiplier with each hit. Just make sure to counter an attack when necessary or your combo will break. Also, try to avoid using any combo attacks or finishers so that your enemies will stay alive longer.
