How to unlock the Perfectionist achievement in Batman: Arkham Origins - Definitive Guide
To unlock all the maps, you need to finish the game and finish 4 (not all) of the campaign maps. Don't bother with this early on because you will need to buy some upgrades for a few of the medals. Warning: one of the challenge maps requires the Sonic Shock Batarang rewarded from Worst Nightmare rank 10
This achievement is super duper easy once you have all the upgrades, especially the disarm X+Y special combo from Bird and the critical strike ability from Shiva.
Powerpyx has provided another awesome video guide, although he seems to have done them the hard way, without using any green bonuses (see below).

Unlike the campaign challenge maps, here, there's every reason to use all the green bonuses before starting the map. Fast health, one kill boomerangs and electric gloves.
For predator maps, just spam LT to boomerangs everyone except for the last few guys you need for the medals, should take no longer than 5 minutes per map.
For combat maps, just turn on electric gloves and press A,A,X (to jump over and hit them from behind) for a super easy win. Once you have a special combo, jump in the middle of the baddies and hit A+X for the bat swarm special for a few thousands points.
Start any fight by hitting () on the first guy in front of you as many times as you can, up to 3.
Counter () before being hit by someone else (avoid guys with knives, so if they attack you first, evade and start again with another group or using them as the target to start the combo) as many times as necessary (up to 3 enemies).
Now that your combo meter is at 4, evade an enemy and hit him in the back. You'll be able to perform a special move (assuming you have all upgrades) so you can do the Bat Swarm (+).
From now on you just have to keep hitting 3 NORMAL/ORDINARY enemies (those who don't block your attacks) and performing the Bat Swarm combo. The higher your combo, the more points you get for executing the move. If you're able to perform, let's say, more than 3 Bat Swarm (the more, the better) without being hit in one round, you'll earn good points. Yeah, you might pass the bonus for performing different variations of moves, but it's still more worthwhile this way.
Another thing to have in mind is that once you're able to use the Shock Gloves (+), instead of having to hit enemies 3x to earn the special move, it decreases to 2, which is really great, specially considering that now none of the enemies is able to block your attacks, so it's pretty much hit , hit , Bat Swarm + and repeat.
When there are only 2 enemies left, you probably won't be able to continue to perform the special, so here's what to do: you can get rid of one of them any way you want or you can work with both of them, without loosing the combo (a bit more risky). Perform the Ultra Stun (++) on him/them (one at a time) and finish the round with Ground Takedowns (+).
As I said at the beginning, some rounds will be more challenging because of the special enemies. In that case, my advice is to take them first or last, depending if you can manage having them around and avoid their charges. Maybe it's best to take some of them down first, so you don't risk hitting them by accident and loosing your combo. It really depends on you. It's possible to perform some beatdowns (,,,...) on these special enemies just to earn special moves and letting go of the beatdown (stopping hitting ) when ready to perform the special move, then rinse and repeat until the special enemy falls. There's always the chance that some enemy wasn't stunned by your last Swarm and will come at you to make you loose the ongoing combo, so be ready to counter.
Anyways, this is the general idea. Using this method sometimes I was able to save a whole attempt that I had messed up, on the very last round.

The custom maps aren't difficult. They are the ranked maps (see medalist for tips, but you can choose any boosters you want to work with. Therefore you can just choose the 3 positives which are health regeneration and one hit batarang, and infinite shock gloves for combat or faster movement for predator. By the time I had got through the ranked and campaign maps, I already knew what to do to get the medals, so I got the medals first, then spammed my batarang near enemies (:l2) to finish it quickly. Activate the shock gloves (+) at the start of combat maps to rack up high combos to get more points (each hit adds two to the combo). After completing all 24 maps to 3 medals, you'll get this.
Custom Challenge maps can be launched at the Main Menu, or at the training terminal at the north end of the Batcave in the Story Mode. Like the Combat Training Maps, these Custom Maps will only unlock through story progression.
The Custom Maps share similarities with the Campaign Challenge maps, in that you must complete all the Combat and Predator challenges with at least on modifier active. Unlike the Campaign Challenges however, they are not stacked into a set of three maps and you do not have to use every modifier.
In fact, I highly recommend choosing any one modifier you are most comfortable with, such as the "Health Replenisher" or "Takedown Projectile" modifier and apply it it every Custom map you work on. Whichever way you go about finishing these 24 Custom Maps, once you have obtained all 72 medals on them, the achievement will finally unlock.
