Story Related Achievement. - Once Arkham Mansion is reached.
To defeat Zsaz
Crouch down
& slowly approach the corner of the doorway where the hostage is being held.
Press
to enter corner cover.
Zsaz is hiding behind the hostage, you must wait until the hostage head moves and you get a clear view of Zsaz's head. When this happens press
to do a Batarang quickthrow.
The achievement pops following the cutscene.

Achievement bugged on me, too. Hard mode. Ran through on Easy to reach that point in the story again and it unlocked as it should that time. Seems like it's probably a good idea to have your cloud save synced to a checkpoint before each story achievement. If it unlocks, great. If not, delete local save and redownload to try again. Saves you having to potentially do another run. Ugh.

Uh ok officially cranky now. The cutscene ended, and the following fight, and the achievement has not popped.

The game promts you through this pretty well. When told to, enter cover at the corner. Watch Zsasz (I don't recommend detective vision here). When he stick his head out, hit him with the Batarang. Walk over and subdue him.
If Zsasz sees you, you will fail. I'm also told that if you wait too long Joker will convince him to kill her anyway. Some have recommended the remote-control batarang, but I found it clumsy compared to a targeted normal one.
