This achievement is story-related and cannot be missed.
You will come to a point in the story that requires you to destroy 3 devices that scramble your hacking tool. 2 of them can be found in the Bowery and the last of them can be found in the underground section that can be found in the Bowery.
You'll have to defeat some goons (armed and unarmed ones) to destroy all of them.
The achievment will unlock right after you have destroyed the 3rd device.

While on the hunt for the cure, your communications get blocked by a Jammer. You will need to destroy 3 antennas in order for your communications to be restored. To disable/destroy them hold down 
Two of them will be guarded by normal thugs above ground with the third guarded by armed thugs in the subway.v
