How to unlock the Perfect Freeflow 2.0 achievement in Batman: Return to Arkham - Arkham City - Definitive Guide
You should attempt this achievement in the Riddler Challenge "Survival of the Fittest", round 3 or 4. Use rounds 1 and 2 to PRACTICE.
Next, 12 moves (actually 13) in one combo sounds intimidating? Not at all. You've got the four singular letter buttons, that's one third of the moves right there:
1. Evade: +. Which you'll be using constantly, alternating with striking.
2. Strike:
3. Cape stun: . After a cape stun, make sure to jump/evade in the opposite direction so you don't accidentally perform an aerial attack and potentially get your combo ruined. Save the aerial attack when there's no other enemies close by.
4. Counter: . Perform this when you see blue lightning bolts over an enemy's head. Don't rely on this too much. Do it once then remember the "jumping constantly" strategy.
Then, you'll knock out another third by just pairing the singular letters. My suggestion is to do the following four moves in the order listed below. Remember, the number in your combo must be yellow to perform the following:
5. Disarm (must be used on an enemy with a weapon): +
6. Combo takedown: +
7. Bat swarm: +
8. Multi ground takedown (must be used when one or more enemies are on the ground, stunned): +
There you go, two thirds of the moves have been done! Next are your wild card moves:
9. Aerial attack: ++ (on the same enemy). For this one, make sure there are no enemies nearby that can attack you and ruin your combo.
10. Combo batarang: + (quickly). Honestly, you can do this one any time your combo number is yellow. It will not consume the yellow.
Next are the grand finale moves. Save these for when you have 2 or 3 enemies left. If you performed everything up to this point and still have too many enemies, use + to get rid of a few goons. For these moves, you can relax on the jumping strategy and just stun enemies with instead to give you breathing room to perform:
11. Combo beatdown: + ( until the enemy is knocked out.)
Then, on the last enemy:
12. Ultra cape stun: ++
and when he's down after the ultra cape stun:
13. Ground takedown: +
In any combat encounter where an enemy is carrying a weapon, or in the Riddler’s Revenge challenge maps, you can earn this trophy. You must use 13 different moves in the same combo in order for it to count. If you get hit once, you have to start over. The moves are as follows, note that you can do the moves in any order, not necessarily as listed:

The best map to attempt this on is the Joker's Carnival DLC map, which you will have by default on PS4. The enemies in this map enter gradually, so you'll have plenty to complete this trophy without being swamped. If you do not have this, Survival of the Fittest (Extreme) on the final round is the best option.
The trophy will unlock as soon as you execute a combo with all 13 moves in it.
