This achievement is story-related and cannot be missed. You will get it for finishing the Mad Hatter side-quest.
At some point in the story, you'll get a call from Alfred. He tells you to get an antidote for your disease. Go to the location and pick it up. After a cutscene, you will find yourself in Mad Hatter's hideout. You have to go through a drug-trip-like fighting scene. Just keep punching the goons and beat down the Mad Hatter whenever he appears.

Just to confirm - this is not story related, it's for a side quest. I've completed the main storyline and did not encounter this.

Side Mission – The Tea Party.
Later on during the story, you will have a cure drop off point. After you have taken this you will get to see Mad Hatter. After the short introduction, you will have to take out his thugs. Once you have done that, the trophy will pop.
