This achievement appears to be glitched for me. It didn't pop on the first playthrough. Then it popped on my second attempt, but somehow doesn't register on Microsoft's servers, as it still appears locked.

Same to me. It did not pop after two attempts. Then I cancelled saving data and started a new playthrough but it didn't pop as well... still locked

Glitched for me as well. Pop up on a second playthrough with a new save file (just to be sure).

Idk just seems to be random with this game. At some point ALL achievements wouldn't unlock

The achievement glitched, so I started again, did every single action possible, when came in Arkham Asylum in moment that Batman talks with Lady Arkham, finally popped.

This trophy is story related and therefore cannot be missed.

After discovering the 'torture chamber' underneath the basement in the Vale residence, you'll need to examine the items and chalk drawings inside it. You'll discover that Vicki was kept in here as a child. After linking the left and right drawings on the wall, you will then make your way to Arkham Asylum. Press to take out the guard. You'll then encounter Lady Arkham (Vicki). The trophy pops shortly afterwards.

Towards the end of this chapter, after having pieced together the clues Alfred left behind at the Vale residence, you will make your way to Arkham Asylum. Once you arrive, the achievement will unlock shortly after finding Lady Arkham.
