Not to take anything away from the other solution as it is a good one and it will work fine, as The Bartlez says you get a A++ rating by collecting all gems and the yarn whilst finishing the level in a certain time.
I wanted to add that these A++ ratings are cumulative across solo, co-op, normal and insane.
I found the easiest way to get this is to play 1-1, 1-2 and 1-3 on solo normal until you get the A++, then do them again on solo insane
Then (if you have a co-op partner) either do all three again on co-op normal and level 1-1 on co-op insane OR play 1-1 and 1-2 on co-op normal then insane
If you don't have a co-op partner you can go for the first 5 on solo normal and then do the same on solo insane, I personally found this easier than going for 10 in one mode when you can do the super easy levels over again (which you have to do for all achievements anyway)

Playing this in co-op with a friend over Xbox LIVE made this achievement very easy. The time allotted in the co-op mode is much more forgiving it seems, so as long as you gather all of the gems and the yarn you'll probably be getting the A++ most of the time. Also, you can split up some of the gathering for a faster run and dying isn't as much of a penalty because you'll resurrect next to your partner often instead of at a previous checkpoint.
All around I agree with this solution's addition to tackle more of these in co-op mode if you can manage that. Solo is fine and very doable, but if you're struggling at all, try it in co-op!

I can confirm that secret levels do count towards the achievement, as I completed the first 9 levels and the 1-7 secret one with A++. The achievement popped when I finished level 9 ! :)

To get an A++ grade in a level you need to collect all of the gems, the ball of yarn and complete the level under par time.
