To get the 3 enemies in play at once, you will have to survive a lot. Once your multiplier hits X3, you'll get this achievement. There's not much else I can say other than just practice (be patient and learn how to dodge fire and kill Super Tanks and Missiles without dying too much).

Helpful tip (for the 1 or 2 players) out there that might attempt this. (It's 2016), not too many on here are playing Battlezone. lol.. Other than myself. The multiplier has nothing to do with points or your score. It has everything to do with how many enemies you kill (missiles, tanks, super tanks), ..so don't let the UFOs distract you, if you can get an easy kill on one, go ahead, but don't go out of your way. I noticed the multiplier had nothing to do with score once I deliberately started just going after enemies and ignoring UFO's for points, I hit the 2x multiplier around 50k, whereas before, I was getting it around 80k or 100k.

Simply don’t kill any enemies and continue dodging their fire, enemies will keep spawning and eventually you will get this achievement. Even if you’re not going for it, it probably will unlock while going for the achievement “Lieutenant.”
