Pretty much the same thing as Carnal Index except just never use Carnal. Charge up and use Heroic Storm when available and kill enemies with it, as long as you don't use Carnal your index should go up the Heroic side after each level.

I remember that bit, but don't remember completely (it was a long time ago). If you have to use carnal for that bit I don't think it will void your chances of getting this achievement as long as you have been doing Heroic on the other levels.
Also not sure if it counts, but when you encounter the Temptress make sure and last LONGER than 2 minutes as that is consider "good" thus being Heroic.

Does anyone have any advice for the confrontation with the Shadow Thanes or whatever? I'm trying to get my heroic skill up, but it seems that I can only kill them in Carnal Mode. I'm not too concerned, but I'd appreciate any advice.

Going for full Heroic upgrades will help you get this along the way just fight using Heroic style more then Carnal and you will achieve this.
