The guide above will give you a description for all collectables, however there is a "glitch" mentioned in the comments after the above guide pertaining to the prisoners. People are having trouble getting the Thane Prisoners to count even after rescuing them and standing right in front of them. After having this problem myself, I discovered that they are only "saved" when you have room for them to join your party. My best advice is to check the guide so you know where and what episodes they are in and be sure to let at least one of your Thanes die without reviving them to make sure the prisoner can join your group and be "saved".
For quick reference there are 5 Thane prisoners.
The first one is in EPISODE 5 in the swamp on a wooden walkway behind a breakable fence. Its before the stone door your men have to move.
The Second is in EPISODE 6 behind a gate that your men need to turn a mechanism to open. 2 runes and the Legendary Shield are also behind the same gate.
The third is in EPISODE 10 after you lower a bridge. Once you cross the bridge, you will find a hidden path to the right. Climb up the wall to find the prisoner.
The fourth is also in EPISODE 10 after climbing a ledge by the boulder. Work your way across the ledge to the other side above the boulder. The prisoner will be here along with a rune.
The fifth and final prisoner is in EPISODE 12 after you come across the glowing wheel that your thanes open. Go through the door and back through a previously explored are down the path that had the torches. The prisoner is at the end of the path with a rune after you open the rolling stone that you couldn't open before.

The achievement is buggy. I got the fourth one but he didn't want to join me the bastard. So I couldn't get it. I have to go through this whole crappy game - maybe one of the worst I've played - anyway.

I had the same experience as Diesskay. That fourth prisoner didn't want to join - just stood there. Does anybody know how to get this without having it glitch?
Well, I'm headed into another playthrough anyways to mop up the remaining achievements. If I can get it I'll try to note what was different and report back, if I find out anything. Hopefully it will let me reload the last save point until I can get this prisoner to join correctly.
