How to unlock the Big Brass Balls achievement in BioShock 2 - Definitive Guide
Save the game all the time. Seriously, all of the time. Before you hack something, save it, before you walk through a door, save it, before you send the last Little Sister in a level through the vent (or harvest her, you evil Subject Delta you) save it. Big Sisters only attack after you've "dealt with" every Little Sister in a level, so you know when they're gonna come and you can set up the battlefield beforehand - proximity mines, trap spears, and mini turrets are your friends.
If you happen to have full health and eve, and full first aid and eve hypos, save the game, 'cause who knows when you'll see more, right?
I know it isn't much of a "solution," but if you save the game a lot, you'll have to re-do less if you end up dying somewhere, and that's really all that getting this achievement is about. Good luck!
When you die just go back to your last save.
If you save often enough, it won't be so bad.
You'll get use to doing it like I did.
The lower the difficulty you go on the less likely you are to die.

Pause the game and go to Options and Turn ON 'Disable Vita-Chambers' (or something similar). Now play the game normally.
If you die you will come back to the main menu and will have to reload your last save (so save often!).
Please note that every time you load a save you will have to go to Options and disable them (don't forget!). (EDIT: This happened to me in the beginning of the game but for some reason the game 'remembered' my settings and kept Vita-Chambers disabled).

Take your time and avoid as much unnecessary conflict as possible. If you are overwhelmed either take cover or run away and use your health kits when needed.
Note: Once you start the game you can turn the Vita-Chambers off through the option menu. Doing this will stop you from making a mistake and using the chambers. Make sure you save on a regular basis.

To unlock this achievement you will need to complete the game without using any Vita-Chambers. Vita-Chambers act as a place to respawn when you die, so if you do die, you don't lose progress you just respawn. To be sure you unlock this achievement, when you first start your game, go into the options via the button and disable the Vita-Chambers. This ensures that if you die you cannot be respawned in one and void the achievement. This then also means you will be relying on your own manual saving, so if you die you will have to load your latest save, so save very often to not lose much progress.
This achievement can be done on Easy difficulty where you are very unlikely to ever die.

so come in as soon as the pause starts and change to disable vita chambers - ON
Against All Odds, don’t make saves on the same safe, during the battle I encountered a technical problem when there are a lot of enemies, especially big sisters and daddies, as soon as you kill someone and the enemy has not yet fallen dead, don’t save right away, so how the game can freeze while saving with an error and the save will end up being corrupted.
